A Step-by-Step Overview of Our Patient Intake & Care Coordination Process

Step 1
Patient gets referred to meet with a Dietitian for Nutrition Counseling from a provider (MD, DO, NP, PA).
Step 2
Our team receives the referral and creates the patient profile.
Step 3
Benefits and coverage are verified.
Step 4
The patient is contacted to schedule an appointment and can choose in-person, virtual, or phone visits.
Step 5
The patient then meets with our Dietitian. A note is sent to the referring provider, and follow-up is scheduled based on coverage.
